NHS Health Checks
Please be aware on 28th June 2023, Public Health Norfolk issued a letter detailing variations to the Primary Care contracted services. Please ensure that you have read, understood and shared these contract variations with all staff within your pharmacy involved with service provision. These changes must be accepted or declined by an authorised person in your organisation by the 28th July 2023.
The NHS Health Check service is now being offered in community pharmacies as well as GP surgeries.
The NHS Health Check is for adults in England between the ages of 40 and 74 is a screening service designed to predict the 10 year risk of a patient developing heart disease so that they may be offered lifestyle advice and intervention where necessary.

Attached are the documents needed for outreach events Registration for off site delivery. Also the new Diabetes filter and Diabetes UK risk score forms Health Checks at Community Events, DiabetesRiskScore_form.

Alcohol Screening
Many pharmacies already include alcohol advice in their Health Checks but this is now officially part of the check. Please find attached the full Audit-C questions AUDIT-C and Full Audit Questions.
Dementia Screening
A new requirement for the NHS Health check is to offer appropriate information on dementia to patients between 65 and 74 years of age.

Reporting of NHS Health Check results is now through PharmOutcomes – a paper template version of the form can be found here: NHS Health Check template form, this must be entered on the online reporting system to receive payment. Contracts, resources and SOPs can also be found on the left hand side of the PharmOutcomes online form once you have logged in.
